Junior Olympic Individual Registration Forms
Competitive J.O. teams (those playing in tournaments) must register individually and will receive $5,000,000 liability insurance and $250,000 accidental injury insurance at no additional charge! ZERO deductible for $12.00 per person and $250 deductible ($8 PP). ALL registering coaches and the manager will receive their own copy of the ASA rule book and each team receives an official ASA score book!
